Yes, as long as your doctor or hospital accepts Medicare, they will accept your Medicare supplement, no matter what medicare supplement company you have.

Most likely yes.  Medicare (Part A and Part B) change their co-pays and deductibles each year.  Premiums for your plan are dictated by those changes and the claims from the prior year of ALL the people who have the same company and plan as you have (likely 10,000+ people).  The claims of the prior year have the most impact.

It depends on your situation when you are applying.  If you are just starting Medicare Part B, your pre-existing condition will not impact your application.
